Antonio is correct, the model release gives right to you filming, this paperwork states the model knew he or she is being filmed and is happy to be filmed, also giving you right to use the content in sale & allowing you to make profit. 

The ID is to prove age, if the said model was masked or wasn´t showing face, no one would know of the age of the said model, hence ID and ask the model to hold the ID so you have 3 items, the 2257, ID and image of said model holding ID.

Yes definitely. Also you need to make sure you keep a copy of the content you shoot along with the Release form. Depending on what country you reside in. I'm in the US and those are the rules here. Plus the declaration / sexual content warning in front of anything you release. Video wise. Goodluck
